Step Two: Complete the Registration Forms

The KES Registration Form is in pdf format. This means that you will need a copy of Adobe Reader to view the document. You can download a free version by following this link:

The Information for All Learners contains information that will guide registration and the roles and responsibilities of the teacher, student and parent (if the learner is under the age of 19 yrs). You can type information directly into the document and print it. Please do not sign the document until all parts of the Learning Plan have been completed and discussed with your teacher at KES.


The Registration Form identifies who you are and the courses that you wish to take. You can enter the information directly into the document and print it when you have completed the form. The tab key on your computer should take you through the form one-box-at-a-time. There may be some items that require additional clarification:

Ab. Anc. is Aboriginal Ancestry and the answers is either Yes or No

Adult St is Adult Student which is a person who was born prior to July 1, 1988 and the answer is either Yes or No.

Alternate Contact Name is usually a parent or guardian in the case of a student who is under the age of 19 and the name of any person that is a personal contact in the case of an Adult Student.

Email Address. Email is one of the ways that we use to contact students and we highly recommend that you have regular access to a computer and check your email on a regular basis. If you are unable to access a computer, please indicate on the form "not available" in the area for Email Address. You can discuss a communication strategy that works for you when you return your forms to the Learning Center.

The teacher will discuss this with you is a section of information that you will provide with your teacher's involvement. If you are using Adobe Reader to enter information, this will appear as a grayed area.


The Learning Plan tells a little about you as a learning and how the course or courses will fit into the overall goals that your have. You can enter the information directly into the document and print it when you have completed the form. The tab key on your computer should take you through the form one-box-at-a-time.


The Course Completion and Communication Schedule is a document that you will complete with your teacher at KES. It will help you and your teacher track the completion of your course(s) and identify key date during which you will discuss and update your learning plan.


Please do not sign the form until you have had the opportunity to discuss the Learning Plan with a teacher at KES or until you have had an opportunity to discuss the teacher's additions to Learning Plan.
